You must have your inbound number set up correctly. Remember the numbers in your contact, lead, and account records must start with the + and country code and have no spaces or punctuation. This is imperative for the unsubscribes to work. The SMS Replies are matched to an existing Lead/Contact/Account by the phone number matching exactly.

This marks a field on the record called Allow Text Messages to "No". If you do not see this field on the Lead/Contact/Account record, have a CRM Administrator customize the form to bring it into view.
